Spinal cord injury treatment through Stem Cell therapy
Coimbatore (PTI): After curing a heart patient through stem cell therapy, a city hospital has treated a 38-year old woman of spinal cord injuries using the same system.

The woman had sustained a spinal cord injury, following a road accident, resulting in paralysis below the neck, without any movement in the limbs, Dr G Bhaktavasalam, Chairman, K G Hospital, told reporters here on Friday.

After performing MRI, CT Scan and X-ray, she was diagnosed to have 'cervical cord injury' with a fracture of the neck bone, he said.

Bone marrow stem cells were processed from the patient and were separated at Nichi-in Centre for Regenerative Medicine, Chennai and flown back to the hospital,he said.

The surgery was performed yesterday and the woman recovering in the ICU, he said.

The cervical cord was already exposed and the bone marrow stem cells were injected into the spinal cord and the neck bones were stabilised using metal plate screws.

The cells taken from the patient had shown that there was no immunological rejection,he said.

Hollywood Actor Christopher Reeve had same type of ailment in 1995 and if the stem cell transplantation treatment would have been available at that time, he could have been saved, Bhaktavatsalam claimed.

      http://ir.alseres.com/releasedetail.cfm?ReleaseID=284951

HOPKINTON, Mass., Jan 07, 2008 /PRNewswire-FirstCall via COMTEX News Network/ -- Alseres Pharmaceuticals, Inc., (Nasdaq: ALSE), announced today that it has concluded enrollment in the Phase I/IIa clinical trial of Cethrin in acute spinal cord injury (SCI). A total of 48 subjects have been enrolled at 9 sites in the United States and Canada. We expect to release periodic updates of the data in 2008 following protocol-specified patient evaluations.

"We continue to be encouraged by the safety and efficacy observations of the trial. The rate and magnitude of improvement of many subjects seems greater than the expected pattern of recovery," commented Dr. Mark Hurtt, Alseres Chief Medical Officer. "The findings of the Phase I/IIa study serve as a strong foundation for the acceleration of our development plans for Cethrin."

Alseres intends to move forward with its previously announced plans for the placebo-controlled, Cethrin Phase IIb trial in acute spinal cord injury at sites in the U.S., Canada, Europe and other countries in 2008. "We believe that Cethrin continues to be the most advanced drug candidate in the clinic for the treatment of acute spinal cord injury in the world. If approved, Cethrin may offer hope for the nearly 11,000 new cases annually in the U.S. alone whose healthcare costs in the first year post-injury exceed $4 Billion," noted Dr. Mark Pykett, President and COO of Alseres. "We are pleased that the enrollment of the Phase I/IIa trial has concluded. Interim reported data have demonstrated encouraging safety and efficacy results over a 30-fold dose range. We look forward to the initiation of the Phase IIb trial in the first half of 2008."

Cethrin contains a proprietary protein that inactivates the action of Rho, a key enzyme that prevents axon regeneration and recovery after SCI. It has been granted Orphan Drug Status in the U.S. In February 2005, enrollment began in an open-label, non-placebo-controlled, dose-escalating Phase I/IIa trial in subjects with acute SCI at sites in the United States and Canada. The trial assessed 5 dose levels of Cethrin from 0.3 mg to 9 mg in both thoracic and cervical injuries.

The trial design includes a number of post-treatment evaluations of the subjects for safety and efficacy for up to one year after treatment. The efficacy measurements assess changes in subjects' sensory and motor functions, as well as overall recovery as measured by the American Spinal Injury Association, or ASIA, Impairment Scale. The ASIA Impairment Scale is used to score subjects within five categories from A to E, with A being complete impairment with no sensory or motor function below the site of injury and E being normal. Grades B through E designate increasing levels of motor and sensory function. The subjects in the Cethrin Phase I/IIa trial suffered a complete thoracic or cervical SCI and were thus classified as an A on the ASIA Impairment Scale at the time of enrollment in the trial.

The 6-month interim data on 37 of these subjects treated with doses of up to 6 mg indicate that 27% of the Cethrin treated subjects improved from ASIA A to ASIA B or better. This is more than 400% greater than the conversion rate seen with the standard of care in a similarly designed study reported by Burns and colleagues in the Journal of Neurotrauma. When subjects with cervical injuries who were treated with Cethrin were analyzed separately, about 46% of the subjects exhibited a conversion rate from ASIA A to ASIA B or better. Moreover, about 18% of subjects overall and 38% of subjects with cervical injuries improved to ASIA C or better over the six months the hallmark of which is recovery of some motor function. In subjects with cervical injuries, the interim efficacy data also suggest that the response rate observed is dose-dependent at the doses up to 6 mg.

To date, the safety and tolerability data for dose levels up to 6 mg indicate that Cethrin appears to be safe and well tolerated. There have been no serious adverse events related to Cethrin as determined by the investigators and the independent Data Safety Monitoring Board, or DSMB. There were two deaths of subjects enrolled in the trial. The DSMB and the clinical investigators attributed the two deaths to causes related to the subjects' initial Spinal Cord Injury, other injuries, or preexisting conditions and not related to Cethrin.

Researchers ask: Is China the sleeping giant of biotech?

PARIS (AFP) — China's biotech sector accounts for just a sliver of its pharmaceutical industry and operates under the cloud of a massive review of licenses issued under a regulator executed last year for accepting bribes.

Even so, experts say, Chinese purveyors of genetically engineered drugs and vaccines -- targeting everything from cancer to Alzheimer's -- are growing at a frenzied pace and are likely to become major actors on the world stage.

"There is no question that the sector is established," said Peter Singer of the McLaughlin-Rotman Centre for Global Health in Toronto who was lead researcher of a study published Monday in Nature Biotechnology.

"What we found really surprising is that in an industry that's only 10 years old, China has innovative products on the market," he told AFP

For their study, Singer and his colleagues selected 22 small- and medium-sized biotech firms from literally thousands operating in the health sector for close scrutiny. They looked for companies that were innovative, both scientifically and in business.

The portrait that emerged is of a dynamic sector that has been growing 30 percent annually over the past decade, reaching a turnover of three billion dollars in the domestic market in 2005.

Yet its activity is dominated by a few big stars and remains dogged by doubts as to its integrity.

It is also a sector led in large measure by "sea turtles" ("hai gui") -- Chinese-born scientists with a decade or two of US or European lab experience under their belts who have come home to found Chinese companies, often with generous backing from the government.

In a market of one billion potential patients, 15 biotech products for health are already on the market, with another 60 in the pipeline, Singer said.

Exhibit A: Gendicine, the first gene therapy product approved after clinical trials anywhere in the world.

A recombinant human adenovirus, Gendicine carries the p53 gene and is administered by injection directly into cancerous tumours in the head and neck, including nasopharyngeal carcinoma.

More than 5,000 patients have received the treatment in combination with radiotherapy, including 400 foreign patients from outside China.

The company became profitable shortly after the launch of the product, approved in 2005 by China's State Drug and Food Administration (SDFA).

That, as it turns out, was not an unimpeachable recommendation.

The SDFA's former director, Zheng Xiaoyu, was executed in July 2007 for accepting bribes in return for issuing drug approvals without proper review.

As a result, a staggering 170,000 licenses granted by the SDFA, especially between 1999 and 2002, are currently under review.

Another company, Shenzhen Beike Technologies, provides a treatment based on umbilical cord and bone marrow stem cells for Alzheimer's, autism, brain trauma, cerebral palsy and spinal cord injury, as well as a dozen other diseases and conditions. The medication is injected directly into the spinal cord of patients.

"There is no need to do clinical trials for this kind of procedure in China," said the study's lead author, Sarah Frew, also a researcher at McLaughlin-Rotman.

"The approach this company is taking is trying the thing on patients rather than doing scientific research," added Singer.

The product has nonetheless been a commercial success, first with Chinese patients and more recently with international patients. When Frew visited the clinic a year ago, there were a dozen foreigners present.

The company's website is filled with glowing testimonials on the effectiveness of the treatment, which costs tens of thousands of dollars.

In most cases the therapies and vaccines developed in China are far less controversial. Indeed, more than 90 percent of products produced in the health biotech sector are biogenerics, with novel products accounting for 3-to-5 percent of the total.

A more recent development are international joint ventures and investment. Shenzhen Chipscreen Biosciences, for example, has developed an anti-cancer drug in cooperation with Huya Bioscience, based in San Diego, California. Once the medication is on the market, the Chinese partner will hold the rights for China, while Huya can lay claim to the rest of the world.

WuXi PharmaTech, which was listed on the New York Stock Exchange in the summer of 2007, is the first biotech service company in China with major foreign clients, including US pharmaceutical giant Merck and Britain's AstraZeneca.

The fact that WuXi has attracted such companies "punctures a little bit the legend that there is no intellectual property in China," said Singer.

Another myth that may soon fall by the wayside is that China can only reproduce what other have done already.

"There is no longer a hegemony on the part of industrialised countries in global biotech innovation," Singer said.
